Who We Support


 At Riverside Care Group, we provide supported living accommodation for adults aged 18 and over who are ready to take the next step toward greater independence. We welcome referrals for individuals who may:

  • Be living with autism, ADHD, learning disabilities, or neurodivergent profiles
  • Be managing mental health conditions, including anxiety, depression, or complex trauma
  • Be care-experienced, including those transitioning from foster care, residential placements, or local authority care
  • Be asylum seekers or refugees in need of culturally sensitive, trauma-informed support
  • Require step-down support from hospital, residential care, or secure settings
  • Be seeking a safe, structured environment to develop life skills, confidence, and community connections


Our homes are designed to be inclusive, empowering, and adaptable—with personalised support plans that reflect each person’s goals, identity, and lived experience.


Our Approach

We take a trauma-informed, person-centred approach to care. That means:

  • We tailor support plans to reflect each individual’s strengths and needs

Referrals from Adult Social Care


We welcome referrals from local authorities, social workers, NHS teams, and community-based professionals. At Riverside Care Group, we understand the importance of timely, transparent communication and collaborative working. Our team is responsive, experienced, and committed to delivering safe, high-quality placements that meet the needs of each individual.


We support adults aged 18+ :

  • Require step-down support from hospital or residential care
  • Be transitioning from foster care or family settings into independence
  • Need a trauma-informed, neurodiversity-affirming environment
  • Benefit from structured support with daily living, emotional regulation, or community integration

Our way:

  • A clear and compassionate referral process
  • Prompt assessments and transition planning
  • Co-produced support plans tailored to each person’s goals
  • Regular updates and partnership working with care coordinators

📋 What to Include


To help us assess suitability, please include:

  • Background information and current diagnosis
  • Risk assessments and care plans
  • Psychiatric or clinical reports (if available)
  • Any relevant safeguarding or placement history


🔍 What Happens Next?

  1. Initial Review – We’ll review the referral and contact you for any further information.
  2. Assessment – A member of our team will arrange a clinical or needs-based assessment.
  3. Visit & Transition Planning – If appropriate, we’ll arrange a visit to the service and begin transition planning.
  4. Placement Offer – A tailored support and risk mitigation plan will be shared with you.
